 + HTTP: modified not to do Keep-Alive on 407 Proxy-Auth. for MSIE

[FIX]
 + general: fixed not to generate a pile of ABORTLOG, PROTOLOG with CFI on Win32
 + porting: fixed making on OS/2 (since 7.8.3)
 + CFI: fixed "--" not to use Filter defined in later alternatives (7.9.3)
 + FTP: fixed SEGV on cache refresh when the original file is removed
 + HTTP: fixed not to wait empty body on Keep-Alive with server
 + HTTP: fixed not to do fflush() before "0 CR LF" (for Netscape6.X)
 + HTTP: fixed not to flush after head for small message
